How it Works

The benefits provided by the Trust Fund are purchased from the Great-West Life Assurance Company, with contributions made, on your behalf, by contributing employers. These contributions are made to the Trust Fund as a result of the Collective Bargaining Agreement. You will be eligible for benefits as described under the "General Provisions".





The Trust Fund is administered by the Board of Trustees who are responsible for the design, administration and funding of the benefits provided by the Trust Fund. In order to assist them in carrying out their duties, the Trustees have appointed various professional people such as accountants, administrators, consultants and lawyers to provide them with specific advice. The Trustees meet regularly with these advisors to review the many issues that arise from the operation of the Trust Fund. The Trustees make all of the decisions regarding the operation of the Trust Fund.

- Vision Care

Full cost of regular eye glass lenses, once in any twenty-four (24) month period (twelve (12) months if twelve (12) years of age or under). Frames - effective April 1, 2005, two hundred dollars ($200.00), including the cost of one eye exam to a maximum of fifty dollars ($50.00), once in any twenty-four (24) month period (twelve (12) months if twelve (12) years of age or under). Contact lenses in lieu of glasses - effective April 1, 2005, maximum two hundred and fifty dollars ($250.00), including the cost of one eye exam to a maximum of fifty dollars ($50.00), once in any twenty-four (24) month period.

- Dental Care

Effective April 1, 2005, reimbursement for covered expenses will be based on the 2003* Ontario Dental Association Schedule of Fees, (*as adjusted from time to time), to a maximum of one thousand five hundred dollars ($1,500.00) per insured individual, per calendar year.
